Things You Need To Know Before Making An Appointment

Visit If you choose to see our Nurse Practitioner, new patient visits are comprehensive and will last 60-90 minutes (some of this time will be spent with a Medical Assistant going over your history). This will provide plenty of time to discuss concerns as well as for our providers to collect critical information that will help in finding the root cause of concern. Each person wanting to establish care needs to book their own new patient appointment.
Insurance Our office does not file insurance as we are a small practice and try to keep our cost affordable. If you have insurance, you may contact your provider and inquire about out of network coverage. Some insurance companies will cover a portion of the visit depending on the plan. If you need a superbill, please request that at checkout each time you visit our office. You are able to utilize your insurance for certain services such as lab work if desired, however you are responsible for knowing the coverage of these per your plan.
Pricing New patient visits are $299. Follow up appointments vary in price, based upon the amount of time your practitioner recommends, varying from $99 to $299. We do not offer payment plans at this time. A credit card deposit for the full amount of the 1st visit is required to reserve your new patient appointment.
Registering for the Patient Portal We ask that you register for the patient portal prior to your initial visit as this portal is a HIPPA compliant space in which messages and lab slips can be sent, information about your visits at the office can be accessed, as well as any supplement protocols and medication information.
Medical History If you have a significant amount of medical history in which you would like to have on record at our office, you will need to send it to us through your patient portal prior to your visit. All other medical history will be taken at your initial visit, and small amounts of paperwork may be scanned the same day and given back to you.
Our Approach As our practice is a blend of traditional and alternative medicine, All of our practitioners have prescribing authority, but are holistically-minded.
Which Provider Should I Choose? Both of our Nurse Practioners see patients of all ages, including newborn, but Kristi is not accepting new patients. Supplements are recommended by all practitioners, and are available at our store at an additional cost.
Supplement Recommendations: We carry a selection of supplements in our office that we personally trust and have carefully researched. If we recommend something we don’t have in stock, we’ll gladly special order it for you, or point you to a reliable place to purchase it. Due to significant variations in supplement quality across manufacturers, we do not provide guidance on products outside of our recommendations.
Payment Policies Well Within Family Practice is a “fee for service” facility. We do not accept ANY form of insurance. Our facility is approved for HSA (Health Savings Account) spending allowances toward any service or supplement purchases. Additionally, you may submit your invoice to your insurance company for reimbursement if allowed. We recommend you check with your HSA/FSA and medical insurance providers PRIOR to making an appointment if this is an important factor for your family - we cannot guarantee coverage. Pre-payment for the full amount of the first visit is required to reserve your new patient appointment. Thereafter, payment is expected at the time of service. We accept cash, check, and all major credit cards. In the event of a returned check, a returned check fee of $35 will be charged, in addition to the bank fees charged to us. We require prepayment for all new patient appointments. New patient visits are $195 - $299. This is a non-refundable fee; be sure you want this appointment BEFORE you make it!
Cancellation, Reschedule, Late & No-Show Policy Existing patients who no-show, cancel, or reschedule an appointment with less than 24 hours of their scheduled appointment time will be charged a cancellation fee of $50. Existing patients who arrive more than 15 minutes late to their scheduled appointment, will be required to reschedule and will be charged a cancellation fee of $50. A credit card is required to reserve your new patient appointment. If you need to re-schedule, we require a minimum of 24 hours notice.
New patients who no-show, cancel, or reschedule an appointment less than 24 hours of their scheduled appointment time will forfeit their ENTIRE deposit.

If you are looking for a new patient appointment and do not currently see one please check back frequently for openings.
